There are very few rationales in this book. If you are a student and need to provide a rationale for every diagnosis, then this book is NOT for you. I don't understand why the word "rationale" is even in the title. I think it's some kind of ploy to get someone to buy the book. The book is good for Nanda Diagnosis listings. It's nice and compact. But if you need individual rationales, you're better off getting a care plan book. The author even tells the reader to do that in the foreword. Too late for me, I bought the book already.
